Andy's House at Home is Hospice Muskoka's community palliative care program, providing compassionate, personalized support for individuals and families navigating the last year of life. Through in-home visits and virtual care, our team helps you receive the care you need while in the comfort of your home.

The Community Palliative Care Pathway provides a coordinated way for individuals and families to access palliative care support, including Andy’s House at Home. Through collaboration with our community partners, we help ensure each person receives the right care, at the right time, based on their unique needs and preferences.

Individuals can access the pathway through a referral from their healthcare provider or by self-referring. Following an assessment, our Nurse Practitioner-led team works alongside patients, families, caregivers, and healthcare providers to develop personalized Goals of Care and connect individuals with the Hospice Muskoka services and community supports that best meet their needs.

Through Andy’s House at Home, our team provides ongoing palliative care support through in-home visits and virtual care. This helps individuals remain in their preferred setting whenever possible, reduces avoidable hospital visits, and provides seamless, compassionate care throughout their journey. Palliative care is covered under OHIP at no cost to patients.
Accessing Community Palliative Care
Referral to Andy's House at Home is through the Community Palliative Care Pathway form submission below. Other options are through OCEAN referral, or calling our Client Care Coordinator at 705-204-2273 (CARE)
